Community Paramedicine Program

What is Community Paramedicine?

Community paramedicine brings supportive resources to the doorstep of community members with non-emergent needs. A team consisting of a paramedic and social worker will respond and spend as much time as needed to help the individual get the appropriate care and find the resources they need.

This program is a partnership between Hendricks Regional Health and the Emergency Medical Services (EMS) agencies of Hendricks County. Services are available through referrals from these partner EMS agencies, a doctor or hospital staff member.

When to Contact Community Paramedicine

In-home services are provided during scheduled appointments. Community paramedicine can help treat non-emergency injuries; provide health assessments and therapy; assist with medication and social services and provide broad access to many community resources. A member of the team will continue to follow up as often as needed to empower the individual to navigate the healthcare system in a way most suitable to their needs.

Here are some common reasons to refer to a community paramedic:

  • In-home health assessments
  • Blood draws for lab results
  • Assisting with management of chronic illness at home
  • Medication reconciliation and education
  • Wellness, healthy eating, resource management education
  • In-home falls assessment
  • Help connecting with other community resources
  • Public health, home health, social services, continuity of care services

How to Contact Community Paramedicine

The Community Paramedicine program is a referral-based service. Referrals are accepted by secure phone or email message at any time from Hendricks County EMS agencies and approved healthcare providers including:

  • EMS staff after an ambulance/patient contact
  • Hendricks Regional Health Emergency Department physician/nurse/case workers
  • Hendricks Regional Health Hospital staff
  • Primary care physicians

If you are an EMS/Fire provider, talk with your EMS Chief about the referral process.
