Our pre-participation physicals dates for our school partners are completed for 2024. Thank you for attending! 

If you missed our dates, we recommend our HRH Immediate Care Centers who offer pre-participation physicals for a $52 flat fee or scheduling an appointment through a primary care physician.

Here are some frequently asked questions/answers if you attended our physical dates:

Where is the physician page of my sports physical?

We ask all parents/students to take a picture of their physician page with their phone at Check-out. This allows you to upload it quickly to the wbsite your school uses for pre-participation physical tracking.

What website does my school use for pre-participtaion physical tracking?

Please refer to your school's athletic website or contact your school.

I cannot find the picture on my phone of the physicianpage to upload it. What do I do? Where can I get a copy?

Please contact your school's athletic department or athletic trainer for a copy.

Why doesn't the school upload the copy of the physician page of the sports physical?

The pre-participation physical tracking websites require an account. This account is something that may or not already have been completed.
